Referral Requirements

Script specifying physical and/or occupational therapy

Diagnosis (preferred)

Accepting all major insurance except for Mainecare.

We do accept UMR (Northern Light Employee Insurance).

Our outpatient clinic is located at Eastern Maine Sports Academy in Veazie, ME. 

Sports Rehabilitation:

  • ACL tears & reconstructions
  • MCL & LCL sprains, tears, and reconstructions
  • Ankle Sprains
  • Muscle Tears and strains
  • Rotator Cuff tears and strains
  • Chronic Tendinitis due to overuse

Orthopedic Rehabilitation:

  • Joint Replacements
  • Muscle reconstructive surgery
  • Ligament tears or sprains
  • Muscle tears or strains
  • Joint, muscle, or tendon inflammation

We also provide Mobile Outpatient Rehabilitation, traveling to our patients in the Greater Bangor Area allows us to provide therapy services in their homes when getting out to a clinic location is difficult. 

Our therapists provide services in:

  • Independent Housing Communities
  • Assisted Living Facilities
  • Skilled Nursing Facilities

We often provide “outpatient home” therapy services after Home Health (Part A) has reached their “maintenance” phase to get the patient to their true potential.
