Referral Requirements

Script specifying physical and/or occupational therapy

Diagnosis (preferred)

Accepting all major insurance except for WellCare and Mainecare.

We do accept UMR (Northern Light Employee Insurance).

We provide Mobile Outpatient Rehabilitation, traveling to our patients in the Greater Bangor Area allows us to provide therapy services in their homes when getting out is difficult. 

Our therapists provide services in:

  • Homes
  • Independent Housing Communities
  • Assisted Living Facilities
  • Skilled Nursing Facilities

We often provide “outpatient home” therapy services after Home Health (Part A) has reached their “maintenance” phase to get the patient to their true potential.

  • Balance and Strengthening
  • Pain Management
  • Post-op/Injury Recovery
  • Independence with Self-care/ADLS
